We Offer
We offer a wide range of home care services as approved by the ODM, ODA, and DODD. All services are based on individualized care plans recommended by waiver case managers, SSA, and/or agency nurses.
Ohio Home Care/PASSPORT Waiver
- Personal Care: services provided as per the person-centered services plan (PCSP) that assist the individual with activities of daily living (ADL) and instrumental activities of daily living (IADL) needs
- Homemaker: services provided as per the person-centered services plan (PCSP) that assist the individual with instrumental activities of daily living (IADL) needs
DODD
- Homemaker/Personal Care (HPC) : Supports a person to be more independent while meeting their daily living needs. HPC advances the individual’s independence within his/her home and community and helps the individual meet daily living needs.
- Shared Living: Ohio Shared Living provides an option for adults with a developmental disability to get the support they need while living with a family member or a caregiver. The person resides with one or more family members or caregivers, and they provide twenty percent or more of the person’s care and support services.
- Non-Medical Transportation (NMT): Non-Medical Transportation assists a person with transportation to their day services, employment services, or places of employment.
- Adult Day Support: ADS helps a person build connections and stay active in their community. While taking part in Adult Day Support Services, participants can also have help with personal care, such as going to the restroom, eating meals, and taking medication.
- Vocational Habilitation: Vocational Habilitation is often called Voc Hab for short. It is a service which provides learning and work experiences, including volunteer work, that help to develop general skills that lead to integrated community employment in a job that matches the person’s interests, strengths, priorities, and abilities.
